If we give you a grant, we will ask you to evaluate your work and fill in an activity report form.

Online activity report form

In an effort to improve the service that we offer to grant recipients we have now introduced the ability to complete this form online. This facility will only be available to you if you used the online portal to submit your application. In order to complete the form online you will need to carry out the following steps:

After you have logged in you will need to click the button to show your 'live' applications. There should now be a link entitled 'Begin activity report'. Clicking on the link will open up your activity report form. You will find the necessary guidance on how to complete the form on the portal.

Hard copy version

If you require a hard copy of the activity report form, please contact our Enquiries team on 0845 300 6200. You can then submit this by post to:

Interim activity report form

For some grants, we may also ask you to submit an interim activity report form in order to release a payment part way through your project.

You can send this by post to the address above or download the electronic version using the link above and email it to: grantsforthearts@artscouncil.org.uk.

Please note that interim activity report forms can only be submitted by post or email - you cannot complete this form online.
